Lady Butterfly is a shinobi encountered by Wolf during his optional revisiting of the past, three years before the main events of Sekiro: Shadows Die Twice. She led the bandits who raided Kuro's home, and Wolf fights and defeats her. She is an optional boss fought through meditation using specific items.

Three years before the main events, Kuro's home was raided by bandits led by the shinobi Lady Butterfly. During this raid, Owl appeared to be killed. Wolf confronted and defeated Lady Butterfly, but was then fatally stabbed by an unknown assailant—later revealed to be Owl, who had faked his death. Wolf only survived because Kuro shared his blood with him, granting him the Dragon's Heritage. Lady Butterfly's role is thus tied to the betrayal that set Wolf on his path.

Through meditation using various items he finds, Sekiro can revisit his past. In this memory, three years ago, Kuro's home was raided by bandits led by the shinobi Lady Butterfly. While Sekiro defeated her, he was fatally stabbed by an unknown assailant and only survived after Kuro shared his blood with him.
